ORDINATION FORM INSTRUCTIONS

First Blank Line: The nominating congregation's name. It can be a new or an established congregation. A new congregation will be added to the assembly as of the time of the nomination.
 
Second Blank Line: The name of the person nominated.
CHECK EACH POSITION THAT CONGREGATION NOMINATES THE PERSON FOR. Many trusted servants have more than one position. THESE THREE LINES SHOULD CORRESPOND TO THE SAME INFORMATION AS IT APPEARS ON THE BACK BELOW WHERE IT SAYS "PRINT FOLLOWING INFORMATION AS IT WILL APPEAR ON CERTIFICATE(S)"

PERSONS ORDAINED:  Check M if person was ordained a Minister. Check CSH is they are becoming a Certified Spiritual Healer. 
Check DES if they are becoming a Diplomat of Earth Stewardship.

Date: What ever the date is of the nomination.
Signature: By someone representing the nominating congregation. It can even be by the person nominated if that person is a member who represents the nominating congregation. This is self explanatory. It must be signed!!!! It should be signed and dated as of the date of the ordination for persons who have been ordained. No one is recognized as a trusted servant who has not signed one of these documents!!!
